Permitted Use
Form Field Extractor may be used for read-only extraction of field names and values from form text supplied by the user.
User Responsibility
Users are responsible for reviewing extracted results before relying on them in any business, support, intake, registration, or administrative workflow.
Limitations
Extraction may be imperfect when form text is ambiguous, incomplete, poorly formatted, duplicated, or mixed with free-form notes.
No Operational Actions
The App does not submit forms, modify forms, approve or reject applications, contact people, send emails, make phone calls, or update external systems.
No Validation Or Decision-Making
The App does not check required fields, validate field formats, verify identity, verify contact information, or make eligibility decisions.
Data And External Systems
The App is read-only, stateless, uses no external APIs, stores no data, and has no side effects.
Output Limitations
Output is limited to declared schema fields, and raw source_text is not returned.
User Review
Users should verify extracted fields and values, especially when the result will be used in a business workflow.
